BOSSI Replacement Card

The Replace Card application is available to Registered Surveyors who require an ID Card replacement due to one of the following reasons:

1. Card lost
2. Card damaged
3. Card stolen
4. Card destroyed.

Before proceeding with the Replace Card application, please ensure you have the following:

• Your personal details exactly match BOSSI’s records – you have not changed your name or address since your previous ID card was issued
• If your details have changed, please update your details by completing an Update Contact Details request.
• A new passport quality photograph in JPG format, which will be used for your identification card.

If all of your supplied information is correct and validated, we will send the card to your correspondence address that we have on record within two weeks.
